The Social Worker provides direct clinical social work and case management services to pediatric patients with complex medical needs and their families.  Responsibilities include supportive counseling, aiding in adjustment to diagnosis and prognosis, assisting in transition to Blythedale and/or the Steven and Alexandra Cohen Long Term Care Pavilion and coordination of the discharge planning process with a multidisciplinary team. The Social Worker reports to the Director of Social Work or designee.   This is a full-time, onsite position with occasional on-call telephone support evenings, weekends and holidays. Coverage is rotating across the department and averages up to 6 weeks per year.
