Under the supervision of the Clinical Supervisor and the Director of Physical Therapy, the Physical Therapist (PT) Per Diem provides independent and professional pediatric physical therapy services. The PT is responsible for evaluating and treating neuro-muscular, functional, and developmental skills to help patients achieve maximum physical functioning in their daily life tasks. This role is integral to the patient's overall medical care and includes education and training for the patient, family, and related disciplines.   Schedule: Per Diem (7.5 hours/day)

The Social Worker provides direct clinical social work and case management services to pediatric patients with complex medical needs and their families.  Responsibilities include supportive counseling, aiding in adjustment to diagnosis and prognosis, assisting in transition to Blythedale and/or the Steven and Alexandra Cohen Long Term Care Pavilion and coordination of the discharge planning process with a multidisciplinary team. The Social Worker reports to the Director of Social Work or designee.   This is a full-time, onsite position with occasional on-call telephone support evenings, weekends and holidays. Coverage is rotating across the department and averages up to 6 weeks per year.
