Overview

Blythedale is seeking a full-time Social Worker who is responsible for the provision of care management services to medically fragile children in the community through the New York State Health Homes Serving Children (HHSC) program.   Responsibilities include coordination of the child’s medical care in collaboration with the family, assessment of and assistance with psychosocial issues that impact the child’s ability to receive consistent medical care, and the creation and monitoring of a child-centered treatment plan.  The Care Manager will also assist the child and family with transitions through the continuum of care including hospital and emergency room admissions and discharges. This position reports to the Care Coordination Programs Supervisor.

Responsibilities

  • Collect detailed medical and psychosocial information through direct meetings with the child, family, and providers.
  • Complete the CANS-NY and other required assessments thoroughly and in a timely fashion as per NYSDOH HHSC program guidelines.
  • Develop child-centered plan of care in conjunction with the family and the treatment team that have specific goals and time frames.
  • Collaborate with clinical service providers to assess potential barriers to care and work with the family to find solutions to ensure quality care.
  • Explore and recommend community resources, promoting choice whenever possible.
  • Complete documentation as required by Blythedale, the lead Health Home, and NYSDOH HHSC program regulations.
  • Regularly monitor the plan of care for progress toward goals and update as needed in a timely fashion.
  • Coordinate regular team meetings with the child and family, including providers either directly or through feedback gathered prior to the meeting.
  • Interact with Medicaid managed care plan as needed to ensure proper authorization for needed services and promote continuity of care.
  • Travel to the child’s home, school, a provider’s office, or other location of the family’s choice monthly as per program acuity requirements, or more often if warranted.
  • Complete required NYSDOH HHSC program trainings and trainings required by the lead Health Home and/or Blythedale in a timely fashion.
  • Comply with all applicable NYSDOH HHSC program and lead Health Home standards regarding identified quality metrics.
  • Allow flexibility within scheduled hours to accommodate a child or family’s appointments and service needs
  • Keep an accurate record of travel outside the building.
  • Participates in other activities assigned by the Care Coordination Services supervisor or designee.

Qualifications

  • Previous experience working with children and families and/or pediatric healthcare is preferred.
  • Strong communication and writing skills required.
  • Ability and means to travel on a frequent basis for client visits and other meetings as required by HHSC program regulations.

Education, Licensure, and Certification

  • New York State Social Work License required.
  • Master’s degree from an accredited social work program is required.
  • Bilingual English/Spanish preferred.
  • Certification in CANS-NY administration preferred.

  • Ability to transfer children and equipment.
