Overview

A Pediatric RN Discharge Planner is responsible for coordinating the medical aspects of discharge planning for both inpatients and Steven and Alexandra Cohen Long Term Care Pavilion (LTC) residents.  They will collaborate with the assigned social worker, patient/family educator, therapists, and other disciplines around issues related to non-medical discharge factors, including education, caregiver training, rehabilitation needs and equipment, and any behavioral health support needed post-discharge. The Pediatric RN Discharge Planner reports to the Director of Social Work and Care Coordination.

Responsibilities

  • The Pediatric RN Discharge Planner makes referrals to homecare agencies, DME suppliers, outpatient therapy sites, health homes, and long-term care facilities as indicated by the treatment team and family.
  • Provide support in understanding what is possible under the patient/resident’s insurances in their geographic area per services determined by patient/resident/family’s choice.
  • Collaborate with the patient/resident’s insurance care manager and/or company as necessary to support continued stay or advocate for discharge needs.

Qualifications

  • Minimum 2 years of experience in a healthcare setting; familiarity with the pediatric patient population preferred.
  • PRI and Screen Certification preferred.
  • Experience with Meditech or another EMR is preferred.
  • Bilingual English/Spanish is desirable.

Education, Licensure, and Certification

  • A Bachelor of Science in Nursing from an accredited school is required.
  • New York State licensure as a Registered Nurse is required.

Physical Requirements

  • Occasional prolonged standing/walking and ability to lift up to 50lbs.
  • Position involves regular time on the patient/resident units.
